Iron for Heartburn & Acid Reflux: Does It Work?
Does Iron (Loha) help with heartburn and acid reflux? This is one of the weaker links in our knowledge graph, and it comes with a twist: the same classical text that describes iron's cooling nature also warns that badly prepared iron can cause acidity.
Ayurveda calls heartburn Amlapitta, literally "sour Pitta", a build-up of the hot dosha in the stomach that pushes upward. Processed iron is cold in potency (Sheeta Virya) and sweet after digestion (Madhura Vipaka), and that cooling profile suits heat-driven Pitta problems in principle.
But iron is never taken raw in Ayurveda. It is used only as Loha Bhasma, an ash made by purifying the metal through repeated quenching in liquids (Nirvapa Shodhana) and then incinerating it in controlled heatings (Puta). The Bhavaprakash Nighantu (Varga 7) warns that improperly prepared iron ash causes constipation, acidity and liver damage.
The honest answer: iron is not a front-line heartburn remedy. Its classical home is anemia, liver and spleen disorders. If you have reflux, it is a practitioner's decision, and active ulcers or severe acidity rule it out.
How Iron Helps with Heartburn & Acid Reflux
Heartburn in Ayurveda is excess Pachaka Pitta, the digestive heat that governs stomach acid, turning too hot and sour. Here is what iron's classical profile says about that picture, both for and against.
The cooling side
Iron tastes bitter and astringent (Tikta, Kashaya), is light and dry (Laghu, Ruksha), cold in potency and sweet after digestion. Bitter and astringent are two of the tastes Ayurveda uses to calm Pitta, and a cold potency counters heat directly. That is the basis for the link to heartburn.
The acidity warning
The Bhavaprakash Nighantu names acidity and constipation as the effects of poorly made iron ash. For someone whose main complaint is acid, that makes quality the deciding factor rather than a detail. Active peptic ulcer disease and severe acidity are listed among the situations where iron should not be taken.
Where iron does fit
The Bhavaprakash lists processed iron as rejuvenating (Rasayana), strength-giving and the chief remedy for anemia (Pandu). When reflux occurs in someone who is also iron-deficient, a practitioner may weigh these together. Studies cited in our anemia sources report that iron ash causes fewer stomach side effects than ferrous sulfate, but our sources hold no studies on iron for heartburn.
How to Use Iron for Heartburn & Acid Reflux
Iron is not something to start on your own for heartburn. It is used only as a finished, processed medicine, chosen and dosed by a qualified Ayurvedic practitioner, and never as raw metal, filings or rust.
| Form | Range used under supervision | Relevance to heartburn |
|---|---|---|
| Iron ash (Loha Bhasma) | 125 to 250 mg twice daily (anemia sources) | Only if a practitioner judges it suitable; avoid with active ulcer or severe acidity |
| Raw iron, filings or rust | Not for use | None |
Vehicle and timing
The vehicle a medicine is taken with is its anupana. For iron ash, our sources give honey and Amla juice, and advise keeping tea, coffee and milk away from the dose.
Watch for warning signs
If heartburn or constipation appears or worsens on an iron preparation, report it to your practitioner. These are the side effects the classical text names.
What usually comes first for heartburn
For reflux itself, our heartburn guide starts with Avipattikar Churna, Shatavari and Licorice, plus a cooling diet: less spicy, sour, fermented and fried food, little alcohol or coffee, and an early evening meal.
Frequently Asked Questions
Can iron cause heartburn?
It can. The Bhavaprakash Nighantu warns that improperly prepared iron ash causes constipation, acidity and liver damage. Quality and practitioner supervision are essential.
Should I take Loha Bhasma if I have acid reflux?
Only if a qualified practitioner prescribes it after assessing you. People with active peptic ulcer disease or severe acidity should not take iron.
Can I take Ayurvedic iron with prescribed iron tablets?
Not without telling your doctor and practitioner. Two iron sources together can push your total dose too high.
What works better than iron for heartburn?
Cooling herbs are the usual first choice. See Licorice for heartburn, Shatavari for heartburn and Amla for heartburn.
Recommended: Start Iron for Heartburn & Acid Reflux
If you came here looking to use iron for heartburn, the simplest starting point is not iron itself.
Best first step: treat the heartburn with the classical anti-acid formula, Avipattikar Churna, 3 to 6 grams with cool water 20 to 30 minutes before meals, plus a cooling, Pitta-calming diet. Iron ash (Loha Bhasma) belongs only to a practitioner's prescription, usually when anemia is also present.
Kitchen version: cooking in a cast-iron pan adds food-level iron to meals without any of the risks of a processed metal medicine.

Do not take iron with active ulcers, severe acidity or iron overload disorders, and never self-prescribe Bhasma.
Safety & Precautions
Iron is the one entry in our herb library where safety comes before everything else. It is a metal, and in Ayurveda it is only ever used after purification and incineration into ash. Never try to prepare or swallow raw iron, filings or rust at home.
Poor preparation is the main danger
The Bhavaprakash Nighantu warns that improperly prepared iron ash (Loha Bhasma) can cause constipation, acidity and liver damage. Quality varies widely between products, so only take an iron preparation prescribed by a qualified practitioner and made by a manufacturer that tests for heavy metals.
Who should not take iron
- Anyone without a confirmed iron-deficiency diagnosis. Iron does nothing for vitamin B12-deficiency anemia.
- People with iron overload disorders such as haemochromatosis or thalassaemia major.
- People with hemolytic anemia, where iron can cause harm.
- People with active peptic ulcer disease or severe acidity.
Interactions and doubling up
Do not combine Ayurvedic iron with prescribed iron tablets without telling your doctor and practitioner. Two iron sources together can push your total dose too high.
Warning signs
Constipation and acidity are the side effects the classical text names, so report them to your practitioner. See a doctor urgently if your hemoglobin is below 7 g/dL or you have chest pain or breathlessness.
